The path to the Ivy league schools is not an easy one. Sometimes even those with a great profile and credentials, fail to convert their dream B-schools.
The most common reasons are:

Choosing un-impactful stories or topics

If a majority of other applicants can say similar things, you should select different stories or topics which are unique

Presenting unfocused, unrealistic and / or unambitious goals

This causes Ad-coms to see failure to articulate proper long term goals and lack of self awareness

Addressing only part of the essay question

When a questions asks 'What matters most and why?' Most of the applicant's only answer the first part of the question and totally ignore answering as to why it matter. More often than not it's the 'why' that matters more. 'Why' defines you more as a person and sheds insight into your underlying values, which is exactly what Ad-coms want to know.

Failing to tailor the application to a school

School fit matters as does aligning your candidacy with what respective MBA admissions committees consider important principles, philosophies and evaluation criteria. So for example, emphasize goal feasibility to Columbia, but not to MIT Sloan, and incorporate Haas' Four Principles into your MBA application
